Ventilator:

A ventilator, sometimes called "respirators" is a machine that provides mechanical ventilation by moving breathable air into and out of the lungs, to deliver breaths to a patient who is physically unable to breathe, or breathing insufficiently.
Mainly these are used in home care, intensive-care medicine, and emergency medicine as standalone units and in anesthesiology as a component of an anesthesia machine. These machines are used to treat patients suffering from conditions including pneumonia, brain injury and stroke.
Mechanical ventilation systems are classified as life-critical systems as failure may result in death. Therefore, precautions must be taken to ensure that they are highly reliable, including their power supply. Mechanical ventilators are carefully designed so that no single point of failure can endanger the patient. They have manual backup mechanisms to enable hand-driven respiration in the absence of power. For example, the mechanical ventilator can be integrated into an anaesthetic machine. They also have safety valves, which open to atmosphere in the absence of power to act as an anti-suffocation valve for spontaneous breathing of the patient. Some systems are equipped with compressed-gas tanks, air compressors or backup batteries to provide ventilation in case of power failure or defective gas supplies. It also incorporate methods to operate or call for help if their mechanisms or software fail. A ventilator is used to push air, with increased levels of oxygen, into the lungs incase of breathing issues. The ventilator also has a humidifier, which adds heat and moisture to the air supply so it matches the body temperature of a patient.
Patients can be given medication to relax the respiratory muscles so their breathing can be fully regulated by the machine. 

Types of Ventilator:

  • Invasive ventilation with a tube inserted into the patient's airway, performed in the intensive care unit in the hospital.
  • Noninvasive ventilation that can be used at home by people with respiratory difficulties as no internal tubes are required.

Uses of Ventilator:

The ventilator does the work of a patient's breathing so their body can rest and recover. Mechanical ventilators are used:

  • As short-term respiratory assistance in surgeries
  • For longer periods in critically ill patients
  • At home for people who have difficulty breathing normally

Types of Mechanical Ventilation:

The two types of mechanical ventilation are

Positive-pressure ventilation:

It pushes the air into the lungs.Positive-pressure ventilators were developed in the early 1950s to treat polio patients with respiratory paralysis. These ventilators blow the air into the lungs of the patient through a tube. They may be invasive or noninvasive.

Invasive ventilation:

  • Endotracheal intubation: the tube is inserted into the airway (trachea) of the patient through the mouth or nose.
  • Tracheostomy: the tube is inserted through a hole made into the airway.

Noninvasive mechanical ventilators contain masks and can be used at home. The three kinds are

  • Continuous positive airway pressure (CPAP): delivers constant and steady air pressure.
  • Autotitrating (adjustable) positive airway pressure (APAP): It changes air pressure according to the breathing pattern.
  • Bilevel positive airway pressure (BiPAP): delivers air with different pressures for inhalation and exhalation.

Negative-pressure ventilation:

It sucks the air into the lungs by making the chest expand and contract. Negative pressure ventilation is very little in use now. These include:

  • Iron lung: It is a metal cylinder which enveloped the patient completely up to the neck.
  • Chest cuirass: It is a small shell which can be strapped to the chest of the patient to create the negative pressure.

Patients receiving prolonged ventilation are anemic, hypoalbuminemic, have moderate severity of illness, and multiple comorbidities. Survival relates to these factors and the underlying illness precipitating respiratory failure, especially COPD. There are seven cause of respiratory failure that include acute respiratory distress syndrome (ARDS)/Pneumonia, chronic obstructive pulmonary disease (COPD), congestive heart failure/coronary artery disease (CHF/CAD), neurological (Neuro), obesity, trauma and post-thoracic transplantation (heart, lung or heart-lung).
